End-Stage COPD

End-stage COPD is the stage where airflow limitation has progressed greatly (usually GOLD 4) and breathlessness can occur even at rest. Treatment focuses on easing breathlessness, preventing flare-ups, long-term oxygen and, when needed, palliative care. In advanced COPD, the focus of treatment is not to reverse the disease but to preserve comfort and function. Optimized inhaler therapy, recognizing and treating flare-ups early, long-term oxygen therapy (in suitable patients) and techniques that ease breathlessness come to the fore at this stage. Pulmonary rehabilitation and nutritional support help preserve muscle strength and endurance.

Palliative care and support

Palliative care is an approach aimed at improving quality of life at every stage of serious illness, not just at the end of life. It focuses on easing symptoms such as breathlessness, anxiety and sleep problems, and on offering psychological and social support to the patient and family. Receiving palliative care does not mean giving up active treatment; the two can be pursued together.
